WHAT YOU NEED:
Birth certificate
Insurance card
Shot records
Documents for diagnosed disabilities
Custody papers
Proof of income (W2, Tax return, previous 12
months check stubs, SNAP, TANF SSI)
Children must turn 3 or 4 years old by
September 1st, 2025
Wednesday, April 30, 2025 @ 1pm - 7pm
Tuesday, May 6, 2025 @ 10am - 3pm
